Housing Case Manager Alice House Position: Housing Case Manager Reports to: Senior Manager, Service Delivery Salary Range: $48,000 - $66,000 Summary The primary functions of this position include: a) Case oversight for clients engaged in the Safe Housing Program. b) Economic Independence Program administration and client engagement; c) Supports positive tenancy relations. Responsibilities Case Management - In partnership with the Senior Manager of Service Delivery and/or Women’s Counsellor, conducts consult and performs intakes with women to determine suitability of Alice Housing to their family needs. - Assesses the emotional, physical, and mental needs of women to determine their housing, food security, economic independence and other skills to support their journey to independent living. - Participates in case conferences with clients and child protection, lawyers or other services required by the client where they need advocacy and support. - Build and oversee a housing case management plan for each client - Support women in building economic independence through job readiness and applications, resource referrals, budget planning, assistance with apartment searching, financial literacy education, and addressing barriers that prevent active engagement in housing and program plans. - Communicate move-out dates and establish move-out plans that oversee and support women in their transition. - Maintain accurate and thorough case management documentation to help inform case management plans and support services. - Oversight of the subsidized housing program including verification of income for working tenants to determine their eligibility and drafting payment plans; with signoff from Management. - Provides support in the development and maintenance of client safety planning. - Provides eviction prevention support to include maintaining household, navigating utility providers, and eliminating rental arrears, confirming co-habitants, and supporting sourcing of solutions. - Facilitates Tenant Meetings and Tenant written communications. - Provides advocacy and community referrals of service Assessment, Counselling & Advocacy ▪ Offers community and system advocacy; takes collaborative approach in the best interest of the client while supporting clients with self-advocacy when at all possible. ▪ Administers approved screening tools and assessments with service users to determine level of risk and/or skills assessments. ▪ Provides client referrals within the agency and amongst external agencies. ▪ Ensures programing and education is responsive to the needs of service users and community while grounded in research and best practices, as it relates to Case Manager programs/education. ▪ Facilitates small group/workshops/educational material development to support housing and economic independence learning amongst past and present clients. ▪ Pursues community education and prevention opportunities specific to improving the economic independence of women who have experienced domestic violence. Administration & Professional Development ▪ Participates in housing promotion, fundraising opportunities, grant pursuits and community awareness campaigns. ▪ Participates in operational protocol development and team decision making as directed. ▪ Compiles and tracks client service access in HIFIS. ▪ Maintains comprehensive case management plans and documentation to support continuity of information and seamless internal communication. ▪ Works with management on client manuals and other housing services policies and procedures. Property Services Coordination ▪ Coordinates with movers, move-in packages, and provides on-site housing support occurring upon move in and move out; including assisting with packing abandoned property. ▪ Supports Property & Facilities Manager with coordinating donations in-kind for units, including storage, value assessment and collecting donor contact information for stewardship purposes. Skills/Qualifications ▪ Bachelor's degree in social work, or a related human services university degree ▪ 5 years’ experience working in a case management capacity with vulnerable populations. ▪ Registration with a professional governing body and/or willingness to work towards registration. ▪ Goal-oriented; reliable and accountable to clients and obtainment of program targets. ▪ Excellent organizational, prioritization and administrative skills. ▪ Strong problem-solving skills with the ability to make decisions using incomplete and/or ambiguous information. ▪ Ability to work in a fast-paced environment and ability to advance multiple priority areas. ▪ Demonstrated expertise in program delivery and facilitation, and advanced group work skills working with children, families, and communities. ▪ Certification in Standard First Aid and CPR, Certification in Non-Violent Crisis Intervention, Suicide Intervention (ASIST), or a commitment to secure these requirements within 3 months of being hired. ▪ Must be cognizant of the issues, concerns, and struggles that are faced by women and their children experiencing family violence. ▪ Demonstrated experience working with crisis intervention; strong de-escalation capabilities and ability to incentivize cooperation. ▪ Must have a developed understanding of Trauma Informed Practice and actively integrate this practice into interactions with clients and stakeholders. ▪ Have extensive knowledge of community agencies and services available to women in Halifax Region. ▪ Hold in strictest confidence client’s personal information and can react confidently as well as competently to any given situation. ▪ Must be resilient and have a balanced holistic approach and outlook towards work and life. ▪ Adhere to professional boundaries and possess a strong work ethic. ▪ Proficiency with Windows, Google, MS Word, Internet and Email.
