Job Description In this valuable role, the Clinical Unregulated Care Provider Supervisor is accountable for the supervision of PSW’s and HSW’s who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care. Respond to and advise/resolve concerns from community staff, patients/families, and other team members. Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones. Initiate patient safety incident investigations and documents to ensure timelines and action plans are met. Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines. Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals. Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems. Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice. What you bring: Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions Minimum of 5 years’ experience as a practicing Registered Nurse or Registered Practical Nurse Comprehensive patient health assessment skills are required Possess an understanding of the principles of community nursing practice Time Type: Full time Join us as we continue our 40-year tradition to help people live better and provide the most compassionate care possible!
