The Nurse Practitioner (NP) is a generalist who offers quality and relevant care across the health continuum and throughout the patient’s lifespan, including health promotion, disease and injury, prevention, curative, supportive, rehabilitative, and palliative care within an interdisciplinary team of health providers. The service model is based on the principles of primary health care with the goal of supporting communities in meeting their health care needs. As a key member of the NB Health Link team, the NP is responsible and accountable for their practice and professional conduct while working in collaboration with physicians to ensure quality and relevant care. As an EM/ANB employee, the NPs are accountable to our patients and families and co-workers by participating in and supporting safety related initiatives as well as acting in a manner that fosters a culture of learning. Safety for patients, caregivers, and workplace are kept the priority. ROLES AND RESPONSIBILITIES Integrates a broad knowledge base and critical appraisal in determining and communicating the diagnosis and the patient’s needs. Applies advanced assessment techniques, critical thinking and clinical decision-making skills when assessing patients. Systematically collects documents and critically analyzes health data by performing a holistic health assessment using multiple tools and sources of data. Completes a prescription accurately and completely according to relevant legislation, standards, and policies. Orders diagnostic imaging tests, laboratory and other tests when clinically indicated and in accordance with the Nurse Practitioner Schedules and program guidelines for Ordering. Promotes health and reduces the risk of complications, illness, and injury for patients. Ensures that diagnostic tests are interpreted, and results are acted upon in an appropriate and timely manner. Monitor the ongoing therapy of patients with chronic stable illnesses. Screen for the presence of chronic illnesses and works in collaboration with physicians who identified presence of chronic illness in patients. Monitors the effectiveness of interventions. Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients. Ability to work independently and as part of the interdisciplinary team including making appropriate referrals. Documents clinical data, assessment findings, diagnosis, plan of care, therapeutic intervention (including consent), patients’ response and clinical rationale in a timely and accurate manner in the patient health record. Maintain a positive working relationship with the interdisciplinary team. Help build protocols and templates in prevention medicine for the program. Responsibilities to support the core provincial program will be the primary task. Other responsibilities may be assigned to support the program. REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS: Bachelor’s Degree in Nursing and successful completion of a postgraduate Nurse Practitioner program in Primary Health Care. Registered as a Nurse Practitioner with the Nurses Association of New Brunswick Current Basic Life Saving certificate and maintain certification. A least 2 years’ experience in primary health care setting in Community Health Centre or Collaborative Practice.; Excellent interpersonal, oral and written communication skills; Demonstrated organizational and leadership skills; Demonstrated ability to work collaboratively with other members of a team toward common goals; Demonstrated ability to function independently in the role with recognized decision-making and problem-solving skills; Demonstrated ability to critically analyze theory and research literature and apply to practice; Demonstrated ability to integrate a determinants of health approach into care delivery; Actively participates as a team member to provide comprehensive care to all patients; Must be able to work all shifts as assigned, maintaining a good attendance record; Adherence to professional ethical principles.
