Job Description
Youth Health Centre Permanent | Full-Time What We Do And Why We Do ItThe Alex Youth Health Centre provides comprehensive health services in a multidisciplinary team through a welcoming, accessible, confidential, non-judgmental and youth-friendly environment. The goals of the Youth Health Centre (YHC) are to provide integrated health, counselling, and social services to reduce barriers for youth aged 12 to 24 and to improve clients’ outcomes in the areas described above.Reporting to the Case Management Team Lead and working collaboratively with the case management team and the broader team of professionals at the Youth Health Centre and at The Alex, the Case Manager carries a caseload and works with each client to develop a goal/care plan to help clients address identified needs, achieve goals, improve outcomes, and move forward with their lives as quickly as is reasonable. This is a permanent, full-time position.What You Will DoProvide professional case management services with fidelity to the pre-identified CM model to an evolving caseload of 30-40 clients, some of whom may be shared with other case managers or other Alex staffDevelop with the client a service plan that is strength based, client centred, and trauma informedArrange, coordinate, and advocate for delivery and access to a range of programs and services designed to meet the client’s needs with the goal of helping them to move forward with their livesMonitor client progress towards their personal goals and improved outcomes and quality of lifeConnect or accompany clients to appropriate services and resources and provide services in the community, in the home, and at The Alex as requiredCoordinate service providers, ensuring a smooth delivery of serviceComplete required questionnaires as scheduled in CiviCRM and collect any other data as directedEnsure timely communication with all service agencies providing service to our clientsProvide crisis support and solution-focused brief counselling and motivational interviewingMaintain client confidentiality and clear boundariesRepresent the program and the agency in the community in a professional mannerWork collaboratively with all team members to achieve excellent service and outcomes for clientsProvide or support group programs and drop-in services as directedAssist with data collection and entry as directedOther duties as requiredWhat You BringUndergraduate degree in Social Work is preferred; a related human services or health services degree or diploma with a minimum of two (2) years of experience will be considered Eligible for registration with their professional discipline in Alberta (e.g., Alberta College of Social Workers) considered an assetA good understanding of Indigenous issues including strong cultural knowledge and ceremonial experienceProficiency in an Indigenous language is considered a strong assetThe ability to collaborate and network within the urban Indigenous community including working effectively with Elders and following traditional protocolsExperience working with vulnerable clientsCommitment to data collectionStrong crisis management and communication skills with the ability to engage clientsAbility to confidently and comfortably administer questionnaires that include sensitive questions and to maintain boundaries in doing soAbility to receive direction and to collaborate with others to achieve agreed upon goalsAbility to work in a changing, sometimes demanding work environmentSkills in assessment, support and intervention, consultation, crisis counselling and advocacyAbility to take initiative and work independently as well as with a teamTraining in any of the following is considered an asset: Applied Suicide Intervention Training, Mental Health First Aid, Nonviolent Crisis Intervention, and Motivational InterviewingA class 5 non-GDL drivers license, clean drivers abstract, and a personal vehicle that can be used for work purposes with $2 million in liability coverage are required First Aid (either Standard/Intermediate or Emergency/Basic) - Level C CPR within 30 days of hire date, certification must be valid for a minimum of 6 months upon hire.A satisfactory criminal record clearance and a child welfare intervention check are requiredKnowledge of the United Nations Declaration on the Rights of Indigenous People (UNDRIP) and the Truth and Reconciliation Commissions (TRC) 94 Calls to Action is an assetWorking EnvironmentMain Alex Building (2840 2 nd Ave SE) 1 FTE (37.5 hours/week)Standard hours 10am – 6pm Monday-FridayContinuous extended periods of sitting, standing, and walking (sometimes up/down stairwells) Occasional driving as part of work duties, in all weather conditionsOccasional physical action, and requires the ability to bend/crouch, reach above shoulder height, kneel/crawl, and carry objects (up to 20 lbs.) throughout shift.Driving On Behalf Of The AlexThis position is required to use your own vehicle for work-related use or drive Alex vehicles for work purposes. Where required, candidates must show proof of a class 5 non-GDL driver’s license, a personal vehicle, a clean 5-year drivers abstract, and $2 million liability insurance to meet our insurance providers requirements.Working ConditionsThis position requires frontline staff in some cases to be exposed to emotionally disturbing events, such as: medical distress, threatened/actual serious injury, violence, abuse, and death; where clients may be experiencing fear, terror, hopelessness, rage, self-harm, and conflict. It is understood and accepted that you will be working in distress/high-stress situations, which may include emergency response where resiliency and stress tolerance and the ability to maintain composure, remain calm and focused is imperative. Note that you may be exposed to bodily fluids.
