Coordinator (Provincial)

Job Summary

As the First Link Intake Coordinator, you are part of a team that provides direct client services to persons living with dementia, and/or to their care partners. You are responsible for managing and processing all referrals received from the community (including health care professionals and self-referrals), entering client information in the system, ensuring all documentation is completed, triaging client needs, recommending applicable programs and completing referrals to ensure timely delivery of client services. This is a 2-year contracted position with possibility of renewal.

Typical Duties and Responsibilities

• Complete intake screening and entering data into the database, assessing clients needs, and triaging accordingly
• Developing and implement triage process for direct and self referrals
• Intake and registration for all programs and services where appropriate. This includes all database documentation, and appropriate communication to program staff
• Support ongoing culture and diversity work including new and existing initiatives
• Send information packages to families and individuals who contact the office for support and information
• Participation in all staff, department, working group and other meetings as appropriate
• Support the Programs and Services department with work under the referral partner strategy
• Maintain confidential client files
• Lead, maintain, update and add resources to internal community resource library
• Work to expand awareness to the general public through various awareness activities
• Participate in provincial and national activities when requested
• Travel as required

Qualifications and Skills

• Must be bilingual in French and English. Unilingual candidates will not be considered.
• A 2-yr diploma in health or social services field and Office Administration, or equivalent;
• Knowledge of Alzheimer’s disease and other dementias, community services, healthcare, the not-for- profit sector
• Community awareness with the ability to build trusting relationships with community partners
• Sensitive to the impact of Alzheimer’s disease on the individual and the family
• Demonstrated knowledge and application of person-centered approaches, problem-solving strategies and confidentiality;
• Must embody the vision and values of the ASNB
• Must be approachable, have solid communication skills and a warm positive attitude with active listening skills
• Must be able to forge strong partnerships with like-minded organizations in the community to build awareness and strengthen the organization
• Ability to work well independently and as part of a team
• Proficient in Zoom and Microsoft programs
• Ability to work outside regularly scheduled hours as required
• Experience working with databases;
• Intake experience an asset;
• Professional phone experience an asset;
• Possess a valid NB driver’s license and be available to travel within the region

Annual Salary: $43,680.00
