Summary:

The Hospital Discharge Assistant will perform the majority of the clerical duties associated with patient home care and Durable Medical Equipment (DME) discharges in BayCare Hospitals. This process will promote teamwork among CTCs and HDAs so that a team goal may be accomplished. Facilitate timely and accurate home care and DME patient discharges to the appropriate care setting and reduce readmissions to the hospital. Perform other duties as assigned.
